The A Care Team

The A Care Home Health Team is a well-integrated unit composed of both in-office and field staff, working in close coordination to provide comprehensive care. The in-office staff handles administrative tasks, manages patient records, schedules appointments, and supports field staff with necessary resources and information. Meanwhile, the field staff, including nurses and caregivers, deliver hands-on medical and personal care directly to patients in their homes. The seamless communication between these two groups ensures that patient needs are met efficiently and effectively, with in-office staff providing crucial support and oversight while field staff focus on delivering high-quality, personalized care.

Medical Doctor

We are fully trained in diagnosing and providing treatments for illnesses & disorders.

Administrator/DPCS

We are dedicated to monitoring and improving the level of care, and assuming oversight of that care through physicians.

Case Managers

We assess clinical needs of patients, oversee research, and advocate for the health and wellness of their patients.

Intake Coordinator

We make sure patient information is correctly recorded and compiled to better keep track of their medical histories.

Medical Record

Our records include but are not limited to patient's history, clinical findings, diagnostic test results, patient's progress and medication

Quality Assurance

We make sure that all products and services conducted are of standard quality for optimal care for patients.

Administrator/DPCS Designee

We are passionate for the coordination of patient care services and other activities relevant to the patient care services furnished including the development of personnel qualifications and the assignment of personnel.
